STUDENT ADMISSION\/ENROLMENT POLICIES AND PROCEDURES<\/h5>\n\n\n\n

I. Required Age for Admission<\/h5>\n\n\n\n

Applicants must meet the required age range set for the following levels at the start of the school year.<\/p>\n\n\n\n

II. Requirements for Application<\/strong><\/p>\n\n\n\n

  1. Report Card (Certified true copy) \u2013 reflecting three grading periods and no grade below 85% in all subjects.<\/li>
  2. Birth Certificate (NSO\/PSA) photocopy<\/li><\/ol>\n\n\n\n

    Note: An original copy is also needed for verification purposes.<\/p>\n\n\n\n

    3. One recent 2 x 2 ID pictures in color<\/p>\n\n\n\n

    4. For Grade 1 applicants only, a Certified copy of Gov\u2019t Permit\/Gov\u2019t. Recognition of the current school attended, SY: 2020-2021.<\/p>\n\n\n\n

    Additional Requirements for Enrolment<\/strong><\/p>\n\n\n\n

    1. Original Report Card (reflecting 1st<\/sup> to 4th<\/sup> grading)<\/li>
    2. Good Moral Certificate<\/li>
    3. Baptismal Certificate (photocopy)- for Catholics only<\/li>
    4. Acceptance Form<\/li><\/ol>\n\n\n\n

         If Applicant is NOT a FILIPINO citizen,<\/strong><\/p>\n\n\n\n

      1. Photocopy of Passport<\/li>
      2. Photocopy of Alien Certificate of Registration (ACR I-Card)<\/li>
      3. ORIGINAL documents<\/strong> must be presented upon submission<\/li><\/ol>\n\n\n\n

        If Applicant HAS a DUAL citizenship,<\/strong><\/p>\n\n\n\n

        1. Photocopy of Philippine Passport<\/li>
        2. Or Photocopy of Certificate of Recognition as a Filipino Citizen<\/li>
        3. ORIGINAL documents<\/strong> must be presented upon submission<\/li><\/ol>\n\n\n\n

          III. Application Procedures<\/strong><\/p>\n\n\n\n

          Interested applicants will:<\/p>\n\n\n\n

          1.    secure application form through this email: smadonlineadmission@gmail.com<\/a><\/p>\n\n\n\n

          2.    place all requirements in a long plastic envelope and submit it to the Registrar\u2019s office for evaluation and checking the documents based on the admission criteria<\/p>\n\n\n\n

          3.    pay the Testing Fee of P500.00 at the Cashier counter.<\/p>\n\n\n\n

          4.    present the Official Receipt to the Registrar\u2019s Office.<\/p>\n\n\n\n

          5.    wait for a text message or email from the Guidance Office regarding the date of online examination.<\/p>\n\n\n\n

            Notes:<\/p>\n\n\n\n

          1.  Once the student passes the online examination, an initial online interview will be conducted by the Guidance personnel and final interview by the Office of the Principal. (Interviews will be done via Google Meet or Zoom<\/li><\/ol>\n\n\n\n

               2.    A copy of Acceptance Form will be given to the parent\/guardian during enrolment.<\/p>\n<\/div>\n\n\n\n
